Texas A&M Physicians’ Family Medicine Residency earns national recognition for patient-centered care

Texas A&M Physicians’ Family Medicine Residency received the National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and long‐term, participative relationships. By obtaining PCMH certification, NCQA recognizes that Texas A&M Physicians is among an elite group that has demonstrated its commitment to quality in health care and a greater than usual commitment to continuity for patients and physicians.

The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork, new team members, and information technology to improve care, improve patients’ experience and reduce costs. This distinction recognizes practices that foster ongoing partnerships between patients and their personal clinicians.

Each patient’s care is overseen by clinician-led care teams that coordinate treatment across the health care system. A growing body of scientific evidence shows that PCMHs are saving money by reducing hospital and emergency department visits, mitigating health disparities and improving patient outcomes, while also improving patient and provider experiences.  In addition to benefiting patient care, the PCMH model allows for enhanced reimbursement, Medicare Access and CHIP Reauthorization Act (MACRA) credit and Maintenance of Certification (MOC) credits.

“A great deal of work by our team of clinicians and staff led to this award and we are both honored and happy to bring a high level of care to our patients and education to our residents in the Texas A&M Family Medicine Residency,” said Nancy Dickey, MD, department head of Primary Care Medicine at Texas A&M College of Medicine and executive director of the A&M Rural and Community Health Institute.

This distinction is valid for three years. Texas A&M met the program’s key elements of program and NCQA standards that align with the joint principles of the Patient-Centered Medical Home established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association.

Texas A&M Physicians is a medical practice comprised of faculty, resident physicians, and staff from the Texas A&M College of Medicine. By serving patients of the Brazos Valley, Texas A&M Physicians combines the mission of education and medicine through mentorship of medical students and residents, while providing valuable clinical services to the community.
